We've now resolved the incident: All active FormAssembly forms should now be reachable again.
We are in contact with the software vendor, FormAssembly, and we also believe we understand the root cause of the issue. There is some coordination that will need to take place for our systems to talk to theirs, to allow the forms to begin identifying our users correctly again.
We have done extensive testing on the technical system that is resulting in errors on our forms, and we are confident there are no issues on Calvin's end -- we have escalated with the vendor via all available means. We will continue to triage the downstream effects of our forms being unavailable and provide updates here.
As of yesterday afternoon, our primary form for the testing center was updated to provide instructions for a workaround process, as this week is time-critical for student requesting testing for finals week.
We have begun investigating an issue with our FormAssembly forms. These forms are used for a variety of applications on campus.
Forms are appearing with an error message, "invalid_response" appears on the page. The form cannot be reached. We are determining how to proceed with our vendor.
