Duo has resolved the error and Calvin's service is restored. If you continue to have issue please reach out to the Helpdesk.
Duo appears to be experiencing an outage, and while MFA requests seem to be making it to peoples' devices, they time out upon approval. There are reports of it happening around the internet. Will update as soon as information becomes available.
